Foundations of Population Health Science and Practice:

Mini Course for Public Health Professionals

Objective

Provide foundational knowledge in population health science and best practices in population health, including the effective design, implementation, and evaluation of population health practice activities to health department workers and health professions students. Foundational knowledge in population health can improve public health practice at the local level and will enable learners to complete more advanced training in population health. 

Learning Objectives

By the end of the six modules, learners will be able to: 

  1. Recognize how social determinants of health are addressed in population health practice. 
  2. Describe essential components of population health and population health practice.
  3. Describe the importance of interprofessional collaboration in population health practice.
  4. Identify methods to ensure that public health initiatives /programs continue to achieve stated goals. 

Target Audience

Public health professionals, students in public health or health professions, allied health and health care providers

Contents

Each module includes:

  • Instructional video that provides introductory content on the population health topic
  • Brief supplemental video to provide a real-world example of the population health topic (Modules 2-5)
  • Case report with reflection questions
  • Resources and references
  • Knowledge check to assess your understanding of the content

Continuing Education

CE for completion of all six (6) modules ended on March 1, 2024. Module users are now able to receive a Certificate of Completion from APTR after successfully completing the post-test (minimum score of 80%).

Training Instructions

  • The course should be taken in order beginning with Module 1.
  • Each module should take approximately one hour to complete.
  • You do not need to complete the entire course in one sitting. 
  • Modules 1-5 include a fictional case report for you to review.
  • Modules 2-5 include a brief supplemental video to provide a real-world example of the population health topic.
  • Module 6 includes content on how population health tools can be applied during a pandemic.
  • To complete the mini course you must take the final Post-Test that assesses knowledge across all six (6) modules.
  • If you require a Certificate of Completion, enter your name and email in the Post-Test.
