 PURPOSEThe Association for Prevention Teaching and Research (APTR) and the Centers for Disease Control and Prevention (CDC) announce the availability of funds to develop a series of interprofessional problem-based case studies in population health for public health and clinical health professions students. The aim is to deepen health professions students’ understanding and ability to apply population health concepts and principles through exploration and analysis of three CDC and HHS initiatives: Health Impact in 5 Years (HI-5), Racial and Ethnic Approaches to Community Health (REACH), and Million Hearts. Each case study should address Population Health Competencies developed by the Regional Medicine-Public Health Education Centers (RMPHEC), topic areas in the Clinical Prevention and Population Health Curriculum Framework, developed by the APTR Healthy People Curriculum Task Force, and leadership and change management concepts and strategies.  FUNDING OPPORTUNITY DESCRIPTIONThe purpose of this Funding Opportunity Announcement (FOA) is to support the development of interprofessional educational case studies and reflection exercises that facilitate learners’ understanding of population health principles, concepts and approaches through analysis of select CDC and HHS evidence-based initiatives implemented in clinical and community settings.  PROJECT REQUIREMENTSThe case study series must include: a. Three (3) one-hour problem-solving case study exercises that address all three identified CDC/HHS initiatives, at least 5 RMPHEC Population Health Competencies, and leadership and change management strategies. Each case study does not need to address all three initiatives and all the identified competencies. The series should collectively address them all. b. A reflection exercise that draws parallels between the three CDC initiatives and how the learner can apply the information to clinical or public health practice to lead change. The following CDC/HHS initiatives and programs should be featured in the case studies: Health Impact in 5 Years (HI-5) initiative – Centers for Disease Control and Prevention Racial and Ethnic Approaches to Community Health (REACH) initiative

 c. Patient/client/population perspective(s) as relevant d. Visual tools such as slide-sets, videos, photos, graphs, or charts (with copyright rights), as applicable. If a video is produced, it should be housed on a publicly-accessible server or on Vimeo.com. e. A detailed learner assessment instrument/quiz with scoring rubric. Each question will have an answer with descriptive explanation. f. A Facilitator Guide describing how the case studies can be integrated into existing coursework or curriculum, and including explanation of relevant issues, topics for further discussion, and recommended resources and readings. 
 ELIGIBILITYPublic health and clinical health professions institutions of higher education in the United States, specifically departments offering education in population health and related disciplines, who are institutional members of the Association for Prevention Teaching and Research at the time of award (membership not required to apply).  CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) This funding opportunity is made available by the Academic Partnerships to Improve Health (APIH) in the Centers for Surveillance, Epidemiology and Scientific Services. APIH focuses on improving the health of individuals and communities through alliances among academic associations and CDC. APIH drives improvement of health outcomes by working within the educational systems for public health and clinical health professions students to enhance teaching of population health concepts; aligning academic approaches (curricula, teaching materials, or methods) and field experiences with ground-level public health priorities and practice needs; fostering interprofessional collaboration and learning in health professional education; strengthening academia's linkages to public health practice and providing opportunities for hands-on experience for students working with communities and public health partners.  |
